Longivew Eye Associates is pleased to participate in most major medical and vision plans. We will happily pre-authorize all benefits prior to your visit as long as you are able to provide our staff with your active insurance information. We ask that you provide us with any vision insurance coverage you may have as well as medical insurance coverage.
If your doctor determines that you require a medical diagnosis or treatment, it may be necessary to utilize your medical benefits.
Unfortunately, due to the amount of time that it takes to authorize benefits, all insurance information must be provided prior to your visit.
